Custom Slaughter and Processing Agreement

This contract is between Bellawood Farm, at 8677 Oregon Rd, Canal Winchester, OH 43110 (Seller) and Buyer (listed below) and effective upon signing this agreement. Lead time: Processing usually takes between 3 and 6 weeks after your order is placed and deposit taken (lead times may vary). Pickup must occur within one week of notification by Bellawood Farm, otherwise additional storage fees apply (see Terms and Conditions below).

• All livestock will be delivered and treated in a humane way while on Bellawood Farm property, both by the Bellawood Farm employees and the livestock delivery personnel. • Bellawood Farm will provide all livestock water during holding, and feed (if held for more than 24 hours) • Bellawood Farm is not liable for animal sickness, accidental injury, or accidental death of an animal while on the farm. • Bellawood Farm is not liable for an inspector condemning an animal or carcass.

Terms & Conditions / Buyer Agreement

The buyer understands and agrees to all of the provisions of the contract, the provisions of the contract are as follows (Customer must agree to all terms below):

Deposit Payment(Required)

Buyer will pay a deposit based on Bellawood Farm’s Custom Processing & Slaughtering Fees. Once animal is slaughted or carcass is purchased by Bellawood Farm, customer deposit is non-refundable.

Cut Sheet: The buyer has communicated their order via a cut sheet or is requesting a standard processing of the animal/carcass (see Standard Cut Sheet)(Required)

Buyer will provide a completed cut sheet prior to Bellawood Farm processing the animal.

• Processing timelines: o Hogs will begin to be processed within 3 days of slaughter o Beef will begin to be processed between 7-10 days of slaughter o Lamb/goats will begin to be processed between 7-10 days of slaughter • If a cut sheet is not provided by the customer before the timelines above, the animal will be processed according to our default cut sheet

Pick Up: Buyers must agree to each notice below(Required)

Buyer understands that custom orders may take between 3-5 weeks to be completed (potentially longer)

Buyers understand that they must pick up their order one week after being contacted by Bellawood Farm

❏ Bellawood Farm has limited fridge and freezer space. If the order has not been picked up one week after notification, a $25/day fee will be added to the buyer’s bill

Post Purchase Disclaimer(Required)

The buyer(s) must inspect products prior to departing from the property. If differences exist between the cut sheet and processed product, Bellawood Farm will work with the buyer to find a suitable solution. The buyer forfeits all rights to request redress after departing the property.
